CONSENT AND WAIVER

I recognize and understand that sports involve risks not encountered in everyday play. With this understanding, and in consideration of River City Jaguars permitting my child to participate in the River City Jaguars program, I covenant and agree to indemnify, hold harmless, and release and forever discharge the team, it’s coaches, referees and other volunteers as are connected with River City Jaguars in any capacity, for any and all damages, losses, claims and/or liability arising out of any and all injury to or caused by my child. With the knowledge and understanding of the foregoing, this is to certify that my child has my permission to play basketball for RCJ program. Further, I hereby authorize any and all emergency medical treatment deemed necessary by any physician, nurse, or other emergency medical personnel. A copy of this authorization shall be as effective as the original.

ELIGIBILITY: A players education shall be based in their home, with curriculum, instruction, and graduation requirement decisions directed by their parents/guardians (some allowable options include homeschool co-ops,part time cottage schools, and dual credit college courses…while still meeting the above definition). Student athletes who have formally graduated from high school or completed their entire high school course of study are no longer eligible to participate. *Exceptions to any eligibility requirement may be granted only with the express approval of the River City Jaguar Board of Directors.

RELEASE OF LIABLITY FOR MINOR PARTICIPANTS

IN CONSIDERATION OF my child/ward being allowed to participate in any way in River City Jaguars basketball related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) to my child from the activities involved in these programs are significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,

1. FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child’s participation; and,

2. I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child’s readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention of the nearest official immediately; and,

3. I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS River City Jaguars Basketball; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to my child’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

4. I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

5. I, the parent/guardian, assert that I have explained to my child/ward: the risks of the activity, his/her responsibilities for adhering to the rules and regulations, and that my child/ward understands this agreement.

- Uniform packaged includes Home/Away Jersey, Shooting Shirt, and Backpack. Sizes will be reconfirmed before order. All players must have a uniform package.

- Please select your appropriate division. If you need financial assistance or a payment plan for registration fees, select “Yes” below and do not select registration as one of your cost options. You must still select and pay for any required uniform items. A member of the basketball leadership team will contact you privately to discuss registration costs and available options.
- Jags are charged a fee for electronic payment acceptance by both Venmo and Stripe. These fees will be applied to your total.
