School Day Out Registration

This forms allows you to register for School Days Out for the period August - December, 2026. In accordance with Colorado licensing, please complete this form for each child you are registering. Parents are responsible for supplying lunch for their students.
School Day Out Fees:

Half Day (either 7:30am - 11:30am or 11:30am - 5:30pm, as available) - $42

Full Day (7:30am, up to 5:30pm) - $73

Registering for School Day Out
- If you HAVE already registered for Before and After School Care for 2026-27, then we already have your details on file. The easiest way for you to register for School Day Out is to email extendedcare@sjdenver.org and let them know which days you would like to register for.
- If you HAVE NOT already registered for Before and After School Care for 2026-27, please proceed to complete the form below.
For the student you are currently registering: have you already completed this registration form (for School Day Out or Before/After Care) during the 2026-27 school year?
You have already completed the necessary registration information, so we won't ask you to do that again. Please email extendedcare@sjdenver.org and let us know who is coming and which days.

I attest that I possess the full, legal authority to enroll this child in the program and to grant all consents and releases executed on this document, and to make binding emergency medical decisions on the child's behalf.
I certify that a complete emergency and enrollment form has been submitted for my child for the 2026–2027 school year. The form completed was for School Day Out or Before and After Care, and was not the enrollment form for the K8 School. I confirm that all emergency contacts, medical permissions, physician details, and parental contact details currently on file with St. John's School remain complete, accurate, and unchanged.
