1. Participation
I give permission for my child to participate in the activities, programs, meetings, outings, events, trips, recreational activities, and other ministry activities organized by Hillside Evangelical Missionary Church - Youth Ministry during the 2026–2027 ministry year.
I understand that activities may take place at the church, at other locations, outdoors, and during transportation to and from ministry activities.
2. Acknowledgement of Risk
I understand that participation in youth ministry activities may involve certain risks, including but not limited to:
- Trips, slips, and falls due to various reasons including weather
- Recreational and sporting activity injury
- Transportation-related risks
- Injuries resulting from participation in games or activities
- Risks associated with travel or activities at locations outside the church
- I understand that reasonable precautions and supervision will be provided, but that accidents and injuries can occur.
3. Assumption of Risk
I voluntarily accept and assume the ordinary and foreseeable risks associated with my child's participation in the ministry's activities.
I understand that this waiver does not remove or limit any responsibility that cannot legally be waived.
4. Release and Hold Harmless
To the fullest extent permitted by applicable law, I release and agree to hold harmless Hillside Evangelical Missionary Church, its directors, officers, employees, ministry leaders, volunteers, and representatives from claims, demands, or expenses arising from my child's participation in ministry activities, except to the extent caused by conduct for which such a release cannot legally apply.
5. Medical Treatment
In the event of an emergency, I authorize ministry leaders or their designated representatives to obtain appropriate medical attention for my child when I cannot be reached promptly.
I understand that reasonable efforts will be made to contact me or the emergency contact provided on the registration form.
6. Personal Responsibility
I agree to inform the ministry of any relevant allergies, medical concerns, dietary restrictions, or other circumstances that may affect my child's safe participation.
I understand that my child is expected to follow the instructions and safety rules of ministry leaders and to behave appropriately during ministry activities.
7. Transportation
Where transportation is provided or arranged by the ministry, I understand that transportation may involve church-owned vehicles, rented vehicles, or transportation provided by approved volunteers or other service providers.
I give permission for my child to participate in such transportation in connection with ministry activities.
8. Parent/Guardian Acknowledgement
By signing below, I confirm that:
I am the parent or legal guardian of the youth participant named above.
I have read and understood this Hold Harmless, Assumption of Risk & Liability Waiver.
I have had the opportunity to ask questions about its contents.
I voluntarily give permission for my child to participate in the 2026–2027 youth ministry year.
I understand that this agreement is intended to apply to ministry activities throughout the 2026–2027 ministry year, subject to applicable law.
