Liability Waiver, Release & Medical Authorization: I understand that participation in the 2026 Ohio State Youth Convention with the Huber Heights Church of God on November 6–9, 2026 may involve risks associated with travel, transportation, recreation, and other group activities. To the fullest extent permitted by law, I voluntarily assume these ordinary and foreseeable risks and release and hold harmless the Huber Heights Church of God, COGO, and their leaders, staff, volunteers, agents, and representatives from claims arising from my child’s participation, except to the extent caused by gross negligence or willful misconduct.
I authorize designated church leaders to obtain or arrange necessary medical or emergency treatment for my child if I cannot be reached or if circumstances require immediate care. I understand that reasonable efforts will be made to contact me and that I am responsible for any resulting medical expenses. I confirm that the medical and emergency information I have provided is accurate and complete to the best of my knowledge.
By signing, I confirm that I have read, understood, and voluntarily agree to this waiver, release, and medical authorization.
