If you are not yet an existing client of Tina Torres Cosmetics, a one-time medical clearance visit with our Nurse Practitioner is required before participating as a model. This visit is completed virtually via Telehealth/ FaceTime and is $50. It allows our medical team to review your health history and make sure you are medically cleared to safely receive treatment.This is a one-time requirement for new TTC patients and does not need to be completed for each future treatment. The $50 medical clearance visit is separate from any model pricing or treatment cost associated with this model call.
I understand and agree. If selected as a model, I am willing to complete the required $50 medical clearance visit if I am not already an established TTC patient.

Any touch-up needed following your model treatment is not included in the model treatment cost.
I understand that touch-ups are not included and that a $100 Touch-Up Office Visit fee will apply if I choose to return for a touch-up.
