Liability Waiver and Release
By registering for and participating in tryouts for the October 2026 – July 2027 season, participants, parents, and legal guardians acknowledge and agree to the following terms:
Assumption of Risk: Athletic activities involve inherent risks of physical injury. Participants voluntarily assume all risks associated with participation in tryouts, including but not limited to physical contact, fall hazards, and facility conditions.
Release of Liability: Team United, Charlotte-Mecklenburg Schools (CMS), their respective officers, directors, employees, coaches, volunteers, and affiliates assume no financial or legal responsibility for injuries, accidents, illnesses, or medical emergencies that occur during or as a result of tryout activities.
Medical Authorization: In the event of an injury or emergency, parent/guardian authorization is given for staff to secure emergency medical care. Any resulting medical costs or claims are the sole responsibility of the participant or their legal guardian.
