Pickleball Participant Waiver & Release of Liability
1. Acknowledgment of Risks
I understand that participation in pickleball and related physical activities involves inherent risks, including but not limited to falls, collisions with other participants, physical contact, equipment-related injuries, weather exposure, and other potential hazards.
I voluntarily choose to participate and acknowledge that I am physically fit, medically able, and adequately prepared to engage in these activities. I knowingly and freely assume all such risks, whether known or unknown.
2. Release of Liability
In consideration of being permitted to participate in pickleball and other activities at Mikas Tennis Academy, I hereby release, waive, and discharge:
Mikas Tennis Academy, its owners, directors, officers, employees, coaches, contractors, volunteers, and affiliates
from any and all claims, demands, damages, losses, causes of action, or liabilities of any kind, whether known or unknown, arising out of or related to my participation, including but not limited to personal injury, illness, property damage, or death, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Indemnification
I agree to indemnify, defend, and hold harmless Mikas Tennis Academy from any and all claims, lawsuits, damages, costs, or expenses (including attorney’s fees) arising out of or related to my actions, participation, or presence at the facility.
4. Medical Authorization
In the event of injury or medical emergency, I authorize Mikas Tennis Academy staff to obtain or assist in obtaining emergency medical treatment on my behalf. I understand and agree that I am solely responsible for any and all medical costs incurred.
5. General Terms
I certify that all information I provide is true and accurate.
I agree to follow all facility rules, safety guidelines, and instructions provided by staff or coaches.
I understand that failure to comply with rules or instructions may result in removal from the facility without refund.
I acknowledge that this waiver is binding and applies each time I participate in activities at the facility.
