Scholarship Funds Request
Use this form to request payment from your Good Soil scholarship. Before submitting a request, please review the Using Your Scholarship section of the Scholarship Portal for information specific to your scholarship type.

Please submit a separate request for each provider or payment. Good Soil will review your request and contact you if any additional information is needed.

Scholarship funds are paid directly to approved providers and are not distributed to families.
