InterRAI CHA Refresher Course Registration Request
Thank you for your interest in attending the North East interRAI Community Health Assessment (CHA) Refresher.

Completion of the Request for Registration Form does not guarantee registration.

Registration confirmations will be sent out by email.

Course Requirements:

1. The participant will have Electronic Assessment Software System: A unique user account in a licensed electronic software system for the interRAI CHA assessment that uploads assessments to the Integrated Assessment Record (IAR)

2. The participant will have a reliable internet connection, a webcam, and a microphone.

3. The participant will have a copy of the required Manuals in time for the course date
