Make sure contact information is correct. Wrong email = no response. You will receive a copy of your submission in your email (allow 10 mins have it be received). You have to be 18 years or older to get tattooed. You cannot get tattooed if you are pregnant or breastfeeding.
This Form is only for Flash Designs located on my website. This is not for custom requests/ custom designs. Custom tattoo requests will be scheduled 1st of the month. Thank you!
