Participation Agreement & Liability Waiver

Dance and movement activities involve physical activity and may carry risks including falls, collisions, strains, sprains, and other injuries. I give permission for the student named below to participate in Mass Mambo Kids activities.I understand that reasonable safety practices will be used, but no physical activity can be guaranteed to be risk-free. I agree to provide accurate information about relevant restrictions or accommodations and to notify the studio of changes.In an emergency, I authorise staff to contact emergency services and my listed emergency contacts. This document is intended as a general participation agreement and should be reviewed by a Massachusetts attorney before being relied upon as a final legal release or waiver. Please complete this form to acknowledge and accept the terms for participation in Mass Mambo Kids dance activities.
