Lehi City Grave Marker Installation Request
Before installation of any new grave marker/headstone in the Lehi City Cemetery, the following information must be completed. By signing this document you agree that you are aware of all grave marker policies, setting/inspection fees. View Fees.

All new grave markers are subject to an inspection fee, size restrictions, and installation requirements. This documentation can be found in the Cemetery Requirements available online at www.lehi-ut.gov/cemetery/

Only two grave markers are allowed per plot, one upright in the headstone row, and one flat.
Note: All markers must be set by appointment only, between 7:00 am and 2:00 pm Tuesday through Thursday. Exceptions can be made by approval of the Lehi City Sexton.
