  • Requirements for Registration are:

    • Be registered, practicing members of St. Christopher's Parish.
    • Please note that attendance at Iona Catholic Secondary School, St. Christopher, St. Luke Port Credit, St. Louis, St. Helen Catholic Elementary School or a family history of receiving sacraments at the parish does not automatically constitute parish registration.
    • Please allot 15 to 20 minutes to complete the form.
    • You will be required to upload Baptismal and First Communion certificates, even if your child(ren) received their sacrament(s) at St. Christopher's. 
    • There will be prompts for you to sign this registration form.

  • The following is a Sacramental Preparation Parental/Guardian Consent & Family Agreement:

    1. Permission to Attend Classes. I, the undersigned, give permission for my child (named above) to attend faith formation classes and related functions of St. Christopher’s Catholic Church.
    2. Permission to Receive Sacraments. I give permission for my child (named above) to receive the sacraments (First Eucharist, Reconciliation, and Confirmation) at the appropriate ages.
    3. Class Work and Assignments. Additionally, as a family we promise to watch the assigned videos and complete all assigned work/assignments for either The First Communion or Confirmation program.
    4. Class and Mass Attendance. Additionally, we understand that a maximum of 4 class absences is allowed each year and that, as Catholics, we are expected to attend Mass every Sunday.

  • Confirmation Sponsor Information and Requirements

  • A Sponsor for Confirmation is a role model of the Faith for the candidate. The sponsor has her/himself received all three sacraments of initiation (Baptism, Eucharist, and Confirmation) and is living in harmony with the Roman Catholic faith. The sponsor supports the candidate being confirmed in her/his commitment to a Christian life.

    Sponsor Requirements (Taken from the 1983 Code of Canon Law Canon 87):

    • Is at least 16 years of age;
    • Is a practicing Catholic;
    • Has received the Sacraments of Baptism, First Communion, and Confirmation;
    • Leads a life in harmony with the Roman Catholic faith;
    • Is not a parent of the candidate; and
    • Sponsor is required to provide a Copy of sponsor’s annotated Baptism certificate (including Confirmation Date – this may be obtained from the Parish where one was baptized), or a copy of their Confirmation certificate.

    This must be submitted by the Confirmation Candidate no later than January 31, 2027.
