This contract is between Forever Love Weddings -By Minister Trevor Clark-Zamoider (the "Business") and you the name below (the "Client")

This must be completed before the start of any services (excluding funerals or consultations), including scripts. A non-refundable deposit must be made within 24 hours at the latest to secure your date. If you have any doubts or questions, please reach out before submitting this contract.

Agreement to Terms
1) NON-REFUNDABLE DEPOSIT: To confirm and secure your requested dates, an initial non-refundable deposit of $50.00 is required to be paid forthwith. This deposit is mandatory and must be paid prior to the commencement of any work, including initial script development. (This will only be returned if Section 4 is applicable).
2) Service Start Time Is Late: If there is any delay in the commencement of your service exceeding a 30 to 60-minute window, a supplementary charge of $50.00 shall be applicable and due immediately prior to the onset of service. Further, in the event that such delay prolongs beyond a duration of one hour, an additional fee of $100.00 shall become applicable and due immediately.
3) Day Of Service: I will arrive to the ceremony site approximately 20-30 minute prior to go over any questions or concerns you may have. Please have the marriage license if applicable and final payment ready for the officiant, if not already paid in full.
4) Minister/Officiant not able to perform: If, for any reason related to the Minister/Officiant, I, Trevor, am unable to personally perform the ceremony on the scheduled date—and the client is not at fault—I will take one of the following actions: A) Issue a full refund of all payments made toward the service. B) Offer an alternative date for the ceremony. C) Due to booking and scheduling needs, assign another qualified officiant from Forever Love Weddings to perform the ceremony, ensuring the same standard of professionalism and service. (If client breaks communication or does not fulfill the terms of this contract no refunds will be issued)
5) Venue Changes: If your venue changes with fewer than 7 days’ notice, a $100 adjustment applies to accommodate the updated travel and scheduling needs.
6) Postponement: We will try to work with you to have the new date booked. If the Minister cannot attend, you will forfeit any money paid towards your service and deposit. You are also accountable for fulfilling the previously arranged financial obligations in their entirety.
7) Cancellation Of Service: In the event of service cancellation (other than officiant initiated under terms 4 & 8), you shall forfeit any and all deposits rendered and be held accountable for fulfilling the previously arranged financial obligations in their entirety.
8) Overall Cost: My compensation for services rendered ranges from $150 to $350 per diem for the ceremony day. To confirm and secure the booking date, a Non-Refundable deposit of $50.00 is mandatory, and the remaining balance must be settled in full PRIOR TO or ON the day of the ceremony. In case of a rehearsal attendance request by the client, a supplementary charge of $50.00 shall apply. The Minister or Officiant retains the prerogative to rescind the service contract at any given time for any motive, such as safety concerns and Non-payment, comprising of remuneration.
9) Agreed upon TOTAL amount of service.
10) Disputes: Any dispute arising out of or in connection with this contract shall, at first instance, be referred to a mediator for resolution. The parties shall attempt to agree upon the appointment of a mediator, upon receipt, by either of them, of a written notice to concur in such appointment. All legal proceedings shall take place in The City Of Providence, RI under their jurisdiction.
11) Release from Liability: I hereby agree, on behalf of myself, my heirs, my personal representatives, the venues, and all guests, to fully and forever discharge and release Forever Love Weddings and its affiliates, including their respective partners, agents, operators, managers, employees, and representatives (“Released Parties”), from any and all claims I may have or hereinafter have for any injury, temporary or permanent disability, death, damages, liabilities, expenses, and/or causes of action, now known or hereinafter known in any jurisdiction in the world, attributable or relating in any manner to the use of Forever Love Weddings’ services, whether caused by the negligence of the Owner or any of the Released Parties or by any other reason. I acknowledge and agree that this Release and Waiver of Liability is intended to be, and is, a complete release of any responsibility of the Released Parties for any and all personal injuries, temporary or permanent disability, death, and/or property damage sustained by me, my heirs, my personal representatives, the venues, and guests. I further acknowledge and agree that it is solely my responsibility to obtain, maintain, and provide any insurance coverage required by any venue, location, or third party associated with my event. I also understand that I am solely responsible for obtaining any required venue licenses or permissions to use the selected location for the service. Forever Love Weddings is not responsible for securing, verifying, or supplying such insurance, licenses, or permissions, nor for any consequences arising from the absence of required coverage or authorization.
12) Client Collaboration and Feedback Timeline: To ensure a seamless and personalized ceremony script, the Client agrees to provide timely feedback on all drafts submitted by the business. Unless otherwise discussed, responses to script drafts are expected within five (5） business days of delivery. Delays in feedback may impact the scheduled completion timeline. I do not impose fees for any script modifications. Kindly ensure that your script is completed and finalized at least 30 days before your scheduled service. Non-compliance with this requirement constitutes a breach of contract, resulting in its termination as outlined in Term 7. This helps me deliver a polished, personalized script without rushing your ceremony.
13) Permission to Feature Ceremony Elements: The business may share excerpts or stylized highlights from the ceremony script, ceremony details, and couple interactions on its website, blog, or promotional materials for inspiration and portfolio purposes. All ceremony scripts and written materials provided by the business are considered “work for hire” under applicable copyright law. Upon full payment, the Client is granted personal use rights. If the Client prefers not to have any portion of their script featured, they must notify the business in writing no later than five (5) business days prior to the ceremony date.
14) Marriage Licenses: It is the responsibility of the couple to obtain their marriage license from the proper authority and provide your own witnesses. I will and can guide you through this process to make it easier on the couple. Not Applicable for Baptisms, Funerals, or Consultations.
15) Addon Requests: Please include in the box below if you request any additional addons. Such as: candle ceremony, sand ceremony, handfasting etc. You are responsible to pick up your supplies.
