PARTICIPANT ACKNOWLEDGMENT
Participation in Dig It! Play Co. activities may include active play, sand or sensory materials, toys, play equipment, trailers, ramps, steps, building activities and other hands-on play. These activities may involve normal risks including slips, falls, collisions, minor cuts, scrapes, allergic reactions and other injuries. By signing below, the parent or legal guardian acknowledges these risks, voluntarily allows the child to participate and agrees to release and hold harmless Dig It! Play Co., its owners, employees, contractors and representatives from claims arising from ordinary participation, to the fullest extent allowed by law.
PARENT / GUARDIAN AGREEMENTS
The child will follow posted and verbal safety rules.
A responsible adult will supervise the child during participation.
Dig It! Play Co. may stop or remove a child from an activity for unsafe behavior.
Known allergies, medical conditions, sensory concerns or activity restrictions that could affect participation will be disclosed before play begins.
Emergency medical care may be requested when reasonably necessary, and related costs remain the responsibility of the parent or legal guardian.
