I understand that the $99 onboarding fee is nonrefundable once paid, including if I withdraw, fail to attend training, do not complete onboarding, do not obtain or renew my PTIN, or am removed for violating company policies.

I understand that I must maintain an active PTIN and satisfy all applicable IRS requirements before preparing federal tax returns for compensation.

I understand that the compensation arrangement is a 70/30 split, with 70% payable to the preparer and 30% retained by DMK Financial Firm, subject to the final written team agreement.

I understand that the exact calculation and payment of commissions may be affected by approved discounts, refunds, chargebacks, rejected bank products, unpaid preparation fees, processing costs, or other adjustments described in the final agreement.

I understand that compensation will only be paid on eligible tax-preparation revenue actually received and cleared by DMK Financial Firm.

I understand that I am not guaranteed a minimum number of clients, tax returns, commissions, refunds, income, or business results.

I understand that I may not promise a client a specific refund amount, approval, advance, funding amount, or refund date.

I agree not to create false information, inflate deductions or expenses, omit income, claim ineligible dependents, or submit a return without the taxpayer’s review and authorization.

I understand that I am responsible for asking appropriate questions, reviewing documentation, identifying inconsistent information, and complying with all applicable due-diligence requirements.

I agree to protect taxpayer information and follow all privacy, data-security, document-retention, and identity-verification procedures.

I understand that taxpayer information may not be shared, copied, downloaded, photographed, stored, or used for any unauthorized purpose.

I understand that all returns may be reviewed for quality control and compliance before submission.

I understand that DMK Financial Firm may reject or decline to file any return that appears inaccurate, incomplete, unsupported, fraudulent, or noncompliant.

I agree to complete all required training and follow the company’s approved pricing, marketing, software, and client-service procedures.

I understand that failure to follow IRS requirements or DMK Financial Firm policies may result in withheld filing privileges, termination from the team, repayment of improperly issued compensation, or reporting when legally required.

I understand that submitting this intake form or paying the onboarding fee does not automatically guarantee acceptance, continued participation, software approval, or employment with DMK Financial Firm.

I understand that my classification, responsibilities, compensation, and working relationship will be controlled by the final written agreement signed by both parties.
