Sole Wallet Membership Enrollment Form

Please use the same email address and phone number you use with Compassionate Soles so your enrollment can be matched to your Square customer profile.

Your monthly membership price and Monthly Care Credit are not the same amount. Following each successful membership payment, the applicable care credit will be added to your Sole Wallet according to program terms.

Monthly Membership Billing
Your selected Sole Wallet membership renews monthly through Square until cancelled according to program terms.

Monthly Care Credit
Following each successful membership payment, the applicable Monthly Care Credit is added to your Sole Wallet:

Essential — $50
Healthy — $85
Compassionate Care — $100

Using Your Wallet
Available Sole Wallet credit may be applied toward eligible Compassionate Soles services.

Carry-Forward Balance
Eligible unused Sole Wallet care credit may carry forward according to program terms.

Successful Payments Only
Failed, declined, reversed, refunded, or otherwise unsuccessful monthly membership payments do not generate additional Monthly Care Credit.

Every active Sole Wallet member is automatically enrolled in Sole Rewards and begins at the Silver Member tier.

Sole Rewards includes two separate components: membership tier status and loyalty points.

Silver Member
All new active Sole Wallet members begin at Silver.

Gold Member
After 12 consecutive paid membership months, qualifying members unlock 5% savings on eligible services.

Platinum VIP Member
After 24 consecutive paid membership months, qualifying members unlock 10% savings on eligible services plus access to Share the Care according to program terms.

Monthly Sole Wallet membership payments do not earn Sole Rewards loyalty points.

Loyalty points are earned separately through qualifying Compassionate Soles services actually rendered.

Membership tenure determines your Silver, Gold, or Platinum VIP status. Qualifying services rendered determine your loyalty-point earnings.

By enrolling in Sole Wallet, I authorize Compassionate Soles to establish recurring monthly billing through Square for the membership level I selected.

I understand that my membership will automatically renew monthly until cancelled according to program terms.

I understand that Monthly Care Credit is added only after a successful membership payment.

I understand that failed or declined payments do not generate additional care credit and may affect my consecutive paid membership tenure.

I understand that I will complete my first membership payment and payment-method authorization securely through Square.

Monthly Membership Payments

Sole Wallet membership payments are generally non-refundable after they have been successfully processed, except when required by applicable law or when Compassionate Soles approves a refund to correct a billing or administrative error.

Monthly Care Credit

Following each successful monthly membership payment, the applicable Monthly Care Credit is added to the member's Sole Wallet. Monthly Care Credit may be applied toward eligible Compassionate Soles services in accordance with program terms.

Cancellation

Members may cancel their Sole Wallet membership to stop future recurring membership charges. Cancellation stops future Monthly Care Credit contributions and future accumulation of paid membership tenure.

Cancellation does not automatically forfeit previously funded, unused Sole Wallet care credit. Any remaining eligible care credit will remain available for use toward qualifying Compassionate Soles services, subject to applicable law and program terms.

Membership Benefits After Cancellation

Membership benefits requiring active membership—including continued membership-tenure progression, Gold or Platinum VIP membership savings, Share the Care privileges, and future Monthly Care Credits—end when membership is cancelled unless otherwise required by applicable program terms or law.

Refunded, Reversed or Disputed Payments

If a monthly membership payment is refunded, reversed, disputed, charged back, or otherwise invalidated after the corresponding Monthly Care Credit has been added, Compassionate Soles may adjust or remove the corresponding credit from the member's Sole Wallet as permitted by applicable law.

If the corresponding care credit has already been redeemed, the member's account may be adjusted to reflect the payment reversal, subject to applicable law.

Cash Redemption

Sole Wallet care credit is intended for use toward eligible Compassionate Soles services and is not redeemable for cash except where required by applicable law.

Billing Errors

Members who believe a membership payment or Sole Wallet balance is incorrect should contact Compassionate Soles promptly so the account can be reviewed and corrected when appropriate.

Qualifying Platinum VIP members receive access to Share the Care after 24 consecutive paid membership months.

Share the Care allows a qualifying Platinum VIP member to designate one eligible family member or friend at a time to receive eligible program benefits according to program terms.

By signing below, I confirm that the information provided in this enrollment form is accurate. I acknowledge that I have reviewed and agree to the Sole Wallet Membership Terms, recurring billing authorization, Sole Rewards provisions, and applicable program policies.

I understand that submitting this form alone does not activate my Sole Wallet membership.

My membership becomes active after recurring billing is established through Square and my first membership payment is successfully processed.
