I, the undersigned parent/guardian, understand that participation in futsal and related training activities involves inherent risks, including but not limited to falls, collisions, injuries, and other physical risks.
I voluntarily authorize my child to participate in Colorado Futsal Academy activities and acknowledge these risks. To the extent permitted by law, I agree to release and hold harmless Colorado Futsal Academy, its coaches, staff, volunteers, facilities, and representatives from claims arising from ordinary risks of participation, except where prohibited by law or caused by their gross negligence or intentional misconduct.
I authorize Colorado Futsal Academy staff to obtain reasonable emergency medical assistance for my child if I cannot be reached. I understand that I am responsible for any medical or emergency expenses incurred.

MEDIA, PHOTOGRAPHY & CONTENT RIGHTS RESERVATION AND EXCLUSIVITY NOTICE

All photographs, videos, digital media, promotional materials, graphics, logos, training content, social media content, and other visual or audiovisual materials created, captured, produced, commissioned, or distributed by COLORADO FUTSAL ACADEMY MAGNUS SCHOOL are the exclusive property of COLORADO FUTSAL ACADEMY MAGNUS SCHOOL, unless otherwise agreed to in writing.

1. Exclusive Media Rights
COLORADO FUTSAL ACADEMY MAGNUS SCHOOL reserves the exclusive right to photograph, film, record, reproduce, edit, publish, distribute, display, promote, and otherwise use all media content associated with its programs, including but not limited to:

Training sessions
Games and competitions
Tournaments
Camps and clinics
Special events
Players and teams
Coaches and staff
Facilities and academy activities
Interviews and promotional appearances
2. Ownership of Content
All official media content produced by or for COLORADO FUTSAL ACADEMY MAGNUS SCHOOL, including photographs, videos, graphics, designs, logos, and promotional materials, remains the intellectual property of COLORADO FUTSAL ACADEMY MAGNUS SCHOOL.

No individual, organization, team, photographer, videographer, media outlet, or third party may reproduce, publish, distribute, sell, modify, or commercially use official COLORADO FUTSAL ACADEMY MAGNUS SCHOOL media without prior written authorization.

3. Unauthorized Use
Unauthorized recording, photography, commercial filming, broadcasting, reproduction, or distribution of official academy content for commercial, promotional, or competitive purposes is prohibited without the express written consent of COLORADO FUTSAL ACADEMY MAGNUS SCHOOL.

Personal photographs and videos taken by parents, family members, and spectators for private, non-commercial use are permitted unless otherwise restricted during a specific event or activity.

4. Use of Player Image and Likeness
Participation in COLORADO FUTSAL ACADEMY MAGNUS SCHOOL programs may include photography, video recording, and media coverage.

The academy reserves the right to use photographs and video recordings of participants for legitimate academy purposes, including:

Website content
Social media
Advertising and marketing
Promotional materials
Event coverage
Educational and training materials
Academy publications
Any restrictions regarding a participant's image or likeness must be submitted to COLORADO FUTSAL ACADEMY MAGNUS SCHOOL in writing before participation.

5. Logo and Brand Protection
The name COLORADO FUTSAL ACADEMY, CFA MAGNUS SCHOOL, associated logos, branding, designs, training methodology materials, and official promotional content may not be used without prior written authorization.

Unauthorized use of the academy's name, logo, photographs, videos, or intellectual property may result in appropriate legal action.

6. Reservation of Rights
COLORADO FUTSAL ACADEMY MAGNUS SCHOOL expressly reserves all rights to its media, photography, video content, branding, intellectual property, and promotional materials.

No rights are granted or transferred to any individual or organization unless specifically authorized in writing by COLORADO FUTSAL ACADEMY MAGNUS SCHOOL.
