Terms and Conditions, Rules & Liability Acknowledgement

I confirm that I have read, understood and agree to the applicable rules, policies, codes of conduct, terms and conditions of Urban Football Academy and, where applicable, Urban Academy Private School. I acknowledge that participation in football, sport, training, matches, physical conditioning and related academy activities involves inherent risks of injury. I understand that reasonable precautions will be taken to safeguard participants, but I accept the ordinary risks associated with participation and agree to comply with all academy and school safety requirements, instructions and disciplinary procedures.

Media, Photography & Publicity Consent

I consent to Urban Football Academy and Urban Academy Private School photographing, recording or filming the student-player during training, matches, tournaments, school activities, events and related programmes. I grant permission for appropriate photographs, video footage and other media to be used for academy or school communication, websites, social media, publications, promotional material, press releases, advertising and reporting purposes, without additional compensation.

Indemnity & Emergency Consent

I acknowledge that the student-player participates in academy, school, sporting and related activities subject to the inherent risks ordinarily associated with such activities. To the extent permitted by law, I indemnify Urban Football Academy, Urban Academy Private School, their directors, employees, coaches, officials, volunteers and authorised representatives against claims arising from ordinary participation, except where loss or injury results from conduct for which liability cannot lawfully be excluded. I further authorise the academy or school to obtain reasonable emergency medical assistance for the student-player where necessary and where I cannot reasonably be contacted beforehand.
