RELEASE & WAIVER OF LIABILITY To the fullest extent permitted by applicable law, I release, waive, discharge, and agree to hold harmless International Soccer Programs Inc. / Schulz Academy, and its owners, directors, officers, coaches, employees, volunteers, representatives, agents, affiliated teams, facility operators, field owners, municipalities, and event organizers from claims for injury, loss, or damage arising out of or related to participation in the tryout or related soccer activities, including claims arising from ordinary negligence where such claims may lawfully be waived.

VOLUNTARY PARTICIPATION & ASSUMPTION OF RISK I understand that soccer tryouts, training, conditioning, scrimmages, games, and related activities involve strenuous physical activity and inherent risks. These risks include, but are not limited to, falls, collisions, muscle or ligament injuries, fractures, head injuries/concussions, heat illness, dehydration, field or weather hazards, serious injury, permanent disability, and, in rare circumstances, death. I voluntarily choose to participate, or permit me or my minor child to participate, and knowingly assume the ordinary and inherent risks associated with participation

MINOR PARTICIPANTS If the player is under 18 years of age, I certify that I am the player’s parent or legal guardian, or that I otherwise have lawful authority to provide the consents and acknowledgments in this document on the player’s behalf. I consent to the minor player’s participation and agree to the terms above on behalf of myself and, to the extent permitted by law, the minor player.

MEDICAL FITNESS & EMERGENCY CARE I certify that the player is physically able to participate and that I have disclosed any known medical condition, injury, allergy, restriction, or other circumstance that may affect safe participation. I authorize Schulz Academy personnel to obtain reasonable emergency medical assistance if necessary. I understand that I am responsible for all medical, ambulance, hospital, or related expenses incurred as a result of participation.
