  • This form can be used to request the following inspection from the Department of Licenses and Inspections (L&I):

    • Reinspections to clear a violation
    • Inspections associated with a business license. 

    Inspections associated with issued construction permits can be requested via eclipse or by using the Interactive Voice Response (IVR) by calling 215-255-0404.

    If you are trying to submit a complaint, you can call 311 or (215) 686-8686 if you are outside of Philadelphia or you can use 311's webportal.

  • Are you scheduling a reinspection to clear a violation?*

  • You will need a copy of your Notice of Violation (NOV) to complete this form. A copy of the NOV is available at atlas.phila.gov.

    An inspector will visit the site to confirm compliance. If the violation has been corrected, you may proactively schedule the final inspection. Please note that if the property is not compliant, a reinspection fee may apply and fines may begin to accrue. Click Next if you wish to continue.
