Unlimited Monthly Adult Pass — $215
Flex passes may be used across any adult class you select on this form, class space permitting. Passes are for the registered participant only and are non-transferable. The unlimited pass covers all adult classes during one monthly billing period.

Adult Participation Waiver
I acknowledge and accept the normal risks associated with dance, movement, acting, rehearsals, and performances. I agree to participate at my own risk and release Inspired Collective Dance Company from liability for any injury or loss.

I authorize Inspired Collective Dance Company to seek emergency medical treatment on my behalf if necessary.
