Dog Grooming Appointment Request

Please review and acknowledge the following consent before proceeding.

Senior pets and pets with medical conditions may experience additional physical or emotional stress during grooming. Puppy Fresh Grooming may modify or discontinue a service if continuing could reasonably compromise the pet's well-being or safety. Comfort and safety take priority over achieving a particular cosmetic result.

If fleas, ticks, or other external parasites are discovered, Puppy Fresh Grooming may stop or modify the service as necessary. Additional cleaning, sanitation, or parasite-related charges may apply when disclosed to the client.

The client agrees to provide a reasonably safe, clean, well-lit area with adequate space for grooming equipment. Children and other household pets should remain away from the immediate grooming area when requested. The client agrees to disclose household hazards and to avoid interfering with handling or restraint while grooming tools are being used. Some pets become more distracted or difficult to handle when they can see or hear their owner. If requested for safety or to help the pet settle, the client agrees to give the groomer reasonable space to work without interruption. Puppy Fresh Grooming may stop an appointment if the environment becomes unsafe for the pet or groomer.

If my pet becomes ill or injured during grooming, Puppy Fresh Grooming will make reasonable efforts to contact me. If I cannot be reached promptly and the groomer reasonably believes immediate veterinary attention is necessary, I authorize Puppy Fresh Grooming to seek veterinary care for my pet. I understand that I am responsible for veterinary expenses except to the extent responsibility is otherwise imposed on Puppy Fresh Grooming by applicable law.

Cancellation / rescheduling policy: Please give at least 48 hours notice to cancel or reschedule. Cancellations made with less than 48 hours notice will be charged 50% of the scheduled service price. That amount is due before your next appointment can be booked. Late / no-show fee: If you are not available at the scheduled time, or the pet cannot be accessed within 15 minutes of the appointment start, the appointment is treated as a no-show and 50% of the scheduled service price will be charged. Arriving late may also mean the groom is shortened or rescheduled, at Puppy Fresh Grooming's discretion.

Payment is due upon completion of services unless another arrangement has been agreed upon in advance. Final price may vary from an initial estimate when additional time or work is required because of coat condition, matting, behavior, or other circumstances discussed with the client.

This permission is optional and is not required to receive grooming services. Puppy Fresh Grooming will not intentionally publish the client's home address or other private identifying information with the pet's photographs.

I certify that the information I have provided regarding my pet's health, medical history, behavior, temperament, and bite history is complete and accurate to the best of my knowledge. I authorize Puppy Fresh Grooming to perform the grooming services requested and understand that professional grooming involves equipment and procedures including scissors, clippers, dryers, grooming tables, restraints, shampoos, and other grooming products and tools. I understand that even when reasonable professional care is exercised, grooming animals involves inherent risks. These can include stress, movement-related nicks or cuts, skin irritation, reactions to grooming products, aggravation of previously existing conditions, and other unexpected reactions or injuries. I voluntarily assume the ordinary and inherent risks associated with having my pet professionally groomed. To the fullest extent permitted by applicable law, I agree to release and hold harmless Puppy Fresh Grooming and its owner/groomer from claims arising solely from the ordinary and inherent risks of grooming, except for claims arising from negligence, gross negligence, intentional misconduct, or liability that cannot legally be waived. I authorize Puppy Fresh Grooming to discontinue or modify the grooming service whenever the groomer reasonably determines that continuing would pose an unacceptable risk to my pet or the groomer. By signing below, I acknowledge that I have read and understand this agreement, have had an opportunity to ask questions, and voluntarily authorize Puppy Fresh Grooming to provide grooming services for my pet.
