Get ready for an incredible evening of making connections, building friendships, and growing in faith! Join us on Thursday, September 17th from 7:00pm – 8:30pm for a community-building game night, awesome music, and a fully loaded DIY popcorn bar featuring all your favorite sweet and savory toppings!

Event Details

Who: Students entering Grades 7–10 (Friends are welcome—you don't need to be a Blessed Trinity parishioner to join us!)

Cost: FREE (Generously sponsored by our parish's Family Life Ministry)

Hosted by: Hopestone Catholic Formation, a ministry known across Ontario for engaging retreats and faith-filled youth experiences.

Grade 11 Students: Want to be a youth leader? Sign up below and plan to come early, at 6:30pm to meet the Hopestone staff so they can show you the ropes!

I, {q3_fullname1}, do hereby consent to have photographs and video taken of my child, {q2_fullname0} for use in any form of media and/or any publicity material produced or printed by the Roman Catholic Episcopal Corporation for the Diocese of Toronto, in Canada (Archdiocese of Toronto). I authorize the photographer/production company to make reproductions of the photograph(s) and video(s) to be used at the full discretion of Blessed Trinity Parish and the Archdiocese of Toronto. I release and forever discharge the aforementioned party and the photographer/videographer.

I, {q3_fullname1} the undersigned parent or guardian of {q2_fullname0}, give my approval for my child to participate in any and all activities prepared by Blessed Trinity Parish, including but not limited to Youth Nights. I understand that by signing this document I accept the Indemnity Waiver included in this form.

Indemnity Waiver
In consideration of the acceptance of the registration and/or participation of my child listed above, for the event/activity prepared by Blessed Trinity Parish, including but not limited to Youth Nights, and sponsorship by Blessed Trinity Parish and the Roman Catholic Episcopal Corporation for the Diocese of Toronto (Archdiocese of Toronto), on behalf of myself, the parent or guardian, my heirs, assigns, executors, and personal representatives, I, the undersigned parent or guardian, release, hold harmless and forever discharge Blessed Trinity Parish staff, caregivers, and volunteers, the Archdiocese of Toronto, its staff, officers, directors, employees and affiliates, from any and all liability, claims, losses, damages, costs or expenses, and waive any such claims against and such persons or organizations, arising directly or indirectly from, or attributable in connection with all parish activities, both on site (Blessed Trinity Parish) and off site, through the sponsorship and organization of the Archdiocese of Toronto and/or Blessed Trinity Parish.

Youth Night Facilitator(s), Parish Staff, and/or volunteer at Blessed Trinity Parish, to release my child from the Youth Night without parental supervision and hereby consent, acknowledge and allow my child to depart the parish premises from the Youth Night without parental or Youth Night Facilitator, Parish Staff or volunteer supervision.

I acknowledge that my child is in Grade 7 or above, and has, in my opinion, the maturity and physical ability necessary to undertake walking home and/or departing the Blessed Trinity Parish premises without supervision.

I further acknowledge that walking home and/or leaving the Blessed Trinity Parish premises unsupervised may be dangerous for my child.

I, on behalf of myself, my family, heirs, personal representative(s), and/or assigns, do hereby expressly and specifically assume any and all risk of injury, illness, death or property damage of any kind resulting from allowing my child to walk home and/or leave the Blessed Trinity Parish premises from the Youth Night and waive, release, absolve, indemnify, and agree to hold harmless Blessed Trinity Parish and/or the Archdiocese of Toronto, its employees, agents, independent contractors, the

organizers, sponsors, supervisors, and participants in the Youth Night for any claim arising out of injury to my child or damage to any property due to my election to allow my child to walk home and/or leave the Blessed Trinity Parish premise without supervision.

I affirm that I am the parent or legal guardian of the above named child and have

the requisite authority and right to execute this consent and waiver form without any other person or third party’s additional consent or approval.

This waiver and consent will remain in effect as long as my child is enrolled in the Youth Night or until I/we notify Blessed Trinity Parish in writing that I revoke this authorization.
