Mask, fins, cylinder, and weight are included. Nitrox is for certified divers.

I accept responsibility for any dive equipment rented or supplied to me by Extreme Dive Fuvahmulah. I agree to pay the reasonable cost of repair or replacement for any loss of, or damage to, equipment caused by my misuse, negligence, or failure to follow staff instructions, excluding normal wear and tear.

I understand that diving activities may include travel by boat to and from dive sites. I acknowledge the inherent risks of boat travel, including boarding and disembarking, rough sea conditions, slipping, falling, equipment movement, and other maritime hazards. I voluntarily assume all risks associated with such boat travel.

Flying After Diving Guidelines from Divers Alert Network (DAN)
The following DAN guidelines are applicable to air dives that are followed by flights at cabin altitudes of 2,000 to 8,000 feet (610 to 2,438 meters) for divers who do not exhibit symptoms of decompression sickness (DCS):

• For a single no-decompression dive, wait at least 12 hours before flying
• For multiple dives per day or multiple days of diving, wait at least 18 hours before flying
• For dives requiring decompression stops, wait at least 24 hours before flying

I understand that the purpose of signing this document is to exempt and release Olige Maldives Private Limited, operating under the business name Extreme Dive Fuvahmulah, together with its owners, employees, crew, designees, and all involved dive boats, whether owned, operated, leased, or chartered (collectively, the "Released Parties").

1. I understand that scuba diving is a hazardous activity with inherent risks and dangers associated therewith, including, but not limited to, risks associated with equipment failure, perils of the sea, as well as acts of fellow divers that could result in my serious injury or death.
By way of my signature, I expressly assume all risks of scuba diving, snorkelling, or swimming, and all associated risks, whether these risks are specifically set forth or not. It is my intention to release the Released Parties from any claim arising from an incident during these activities that results in my personal injury or death.

2. By my signature on this release, I assert that I am physically fit to participate in the activities of swimming, scuba diving, and snorkelling, and I agree by way of my signature that I will not hold any of the released parties or above named individuals, persons, or entities responsible if I am injured as a result of any medical conditions while swimming, Scuba diving and/or snorkeling. I do not have in my possession any illegal drugs, nor am I taking, nor have I recently taken any drugs or medications that could cause an adverse reaction as a result of combining such drugs and/or medication with scuba diving.

3. Prior to diving, I will inspect all equipment to be used to make sure that it is in good operating condition. I will not hold the released parties or anyone else responsible for my failure to inspect my equipment prior to diving or for any equipment failure that may occur.

4. I hereby confirm that I will attend the tiger shark safety briefing before engaging in tiger shark dives or any other shark dives. Furthermore, I request to dive with these sharks with the understanding that I am fully aware of the risks associated with shark diving. Contrary to what is stated in this Release and Waiver Agreement, I have not been given any assurances or explicit promises to the contrary.

5. I hereby confirm that I have been informed of the Maldives' diving restrictions, which include a maximum diving depth of 30 meters, a maximum dive time of 45 minutes, the requirement to wear a dive computer, and no divers allowed on decompression dives.

6. I understand that valid dive insurance is mandatory for all divers.

7. I understand that I have an affirmative duty to plan and carry out my own dive and to be responsible for my own safety. By way of my signature, I expressly agree that I will plan all my dives as no decompression dives and within the safe limits of recreational scuba diving.

8. I am fully aware of and have been trained in the dangers, risks, and hazards of holding my breath while diving in compressed air. I fully agree not to hold the released parties responsible for any such injuries sustained by me.

9. I understand that diving activities may be conducted at remote sites, far from medical facilities and recompression chambers. I further understand that there is no recompression chamber on Fuvahmulah Island. I choose to proceed and assume the related risks. In an emergency, Extreme Dive Fuvahmulah will arrange appropriate assistance consistent with its emergency procedures and available local services.

By way of my signature on this document, it is my express intention to give up my right to sue all individuals, entities, or vessels referred to herein (the "Released Parties"), whether specifically named or not, from all liability arising as a consequence of any act or omission, including, but not limited to, active or passive negligence, including gross negligence. I fully agree to indemnify and hold the Released Parties harmless from any and all liability for personal injury of any sort, including wrongful death, on behalf of myself, my heirs, and assigns, and I expressly assume all risks in connection with swimming, snorkelling, and scuba diving activities.

By way of my voluntary signature, I agree that I have read fully and understand this document in its entirety. I understand that this is a legally binding contract not to sue.
