Please sign below that you adhere to the following policies and waiver:
- Deadline: Forms Must be submitted no later than Wednesday, October 21, 2026 @ 1pm DST.
- Event Setup is between 4:00pm – 5:00pm | Parking Lot #14 - Located south of the Admin. Building
- Canopy Stations Only (no vehicle setup).
- We encourage all our booth participants to stay for the entire event.
- All canopies must be properly weighted down for safety and stability
- There will be no electrical support for canopies.
- Bring your own tables and chairs.
BOOTH ACKNOWLEDGEMENT/WAIVER

I do hereby release and discharge Tuba City Regional Health Care Corporation, its officials, officers, volunteers and employees from any and all claims from injuries, damage or loss, which I may have, or which may occur to me as a result of my participation or attendance in any of TCRHCC program, activities, and/or events.
