EMPLOYMENT APPLICATION / SOLICITUD DE EMPLEO

1. Show up on time at the work site or arranged place. More than 3 unexcused absences will result in immediate termination. It is the employee's responsibility to notify the employer of any absences. A medical certificate must be brought in.
2. Drinking alcohol during working hours or in the workplace is strictly prohibited. Failure to comply will result in immediate termination. The company will not be held responsible for incidents while an employee is under the influence of any substance.
3. Bring your own essential tools to the workplace.
4. Company-owned tools must be properly cared for. Loss or damage due to worker negligence may result in replacement or repair costs as permitted by applicable law.
5. If the worksite is more than a 40-minute drive, one trip will be deducted.
6. Working on Saturdays is mandatory; you will be notified in advance.
7. Personal protective equipment is mandatory for employee safety. Failure to comply will result in immediate removal from the job site.
8. Discussing each other's salaries is prohibited.
9. Start kit includes hard hat, 5 T-shirts, glasses, gloves and earplugs. Replacement cost for a lost or damaged hard hat is $80.00.
10. Sneakers are prohibited; steel-toed shoes are required.
11. Cell phone use is for emergencies only. Three verbal warnings will be given. Continued use during work hours may result in a one-week suspension.
12. RM Concrete Construction, LLC reserves the right to terminate the employment relationship without prior notice.
13. In case of resignation, shirts and hard hat must be returned to receive the last paycheck.

1. Presentarse a tiempo en el sitio de trabajo o en el lugar acordado. Más de 3 faltas injustificadas resultará en terminación inmediata. Es responsabilidad del empleado avisar en caso de faltar. En caso de ir al doctor se necesita traer comprobante del doctor.
2. Queda estrictamente prohibido ingerir alcohol en horas y/o en el sitio de trabajo. El incumplimiento resultará en terminación inmediata. RM Concrete Construction no es responsable de accidentes ocasionados por estar bajo la influencia de alcohol o drogas.
3. Traer herramienta propia y básica al lugar de trabajo.
4. Se deben cuidar las herramientas propiedad de RM Concrete Construction. En caso de pérdida o daño por negligencia, podrán aplicarse costos de reposición o reparación según lo permita la ley aplicable.
5. Si el sitio de trabajo está a más de 40 minutos de distancia, solo se pagará la ida.
6. Trabajar los sábados es mandatorio. Se notificará con tiempo de anticipación.
7. El equipo de protección personal es obligatorio para la seguridad de todos. El incumplimiento resultará en retiro inmediato del sitio de trabajo.
8. No se permite comentar entre compañeros el salario de cada uno.
9. El kit de inicio consta de casco, 5 playeras, guantes, lentes de seguridad y tapones para los oídos. Reemplazar un casco perdido o dañado cuesta $80.00.
10. Botas de trabajo con casquillo son requeridas; no se permiten tenis.
11. El uso del celular es únicamente para emergencias. Se darán 3 avisos verbales; el uso constante durante horas de trabajo puede resultar en suspensión de una semana.
12. En caso de renuncia o terminación de empleo, casco y playeras deben ser regresados para recibir el último cheque.
13. RM Concrete Construction se reserva el derecho de terminar la relación de empleo sin previo aviso.
