Purpose of This Form
This form is used to register new One Church One School partnerships and update information for existing partnerships. The information provided will help the National OCOS office maintain accurate partnership records, reconnect with participating churches and schools, understand current areas of partnership, and communicate future program, resource, training, and funding opportunities. Please provide the most current information available for both the faith-based organizaiton (church) and school. Following submission, the National Office may contact the individuals listed to confirm partnership information and provide next steps.
