· Should more then 1 application be recieved for one SȾÁUTW̱ youth, staff may request proof of custody.

Please do not apply for Children that are not in your custody / care. Custody eligibility can be resolved by providing : Current years CRA / Child tax benefit notification.

I understand that all required information and supporting documentation must be provided before my application can be fully reviewed and processed.
I understand that processing times may vary and may take up to 10 business days from the date a complete application is received.
I understand that this funding is intended as one-time support toward eligible youth extracurricular activity registration costs and is subject to eligibility, available funding, and approval.
I understand that the maximum funding available is $400 per registered SȾÁUTW̱ youth and that submitting an application does not guarantee funding or approval.
I understand that staff may contact me if additional information or clarification is required and that this may affect processing timelines.
I agree to communicate with SȾÁUTW̱ staff in a respectful and appropriate manner throughout the application and processing period.
I understand that harassment, intimidation, threats, or abusive communication toward staff will not be tolerated.
I understand that repeated contact regarding the status of an application will not expedite processing and that staff require reasonable time to complete the review and processing of applications.
