2026 Awards Banquet Reservation Form

The CMHA awards banquet will be held on Saturday, November 14th, 2026. The doors will open for the banquet at 5:30 pm with dinner to be served at 6:00 pm. The banquet will be held following the 2026 CMHA Annual Meeting in the Bach/Beethoven Room at the Bavarian Inn Lodge, One Covered Bridge Lane, Frankenmuth, MI 48734.
