Freddie Ford Family Foundation is having a Trunk or Treat for children with autism and their friends and family. Please let us know if you will be able to make it.

Time:       7:00 - 9:00 p.m.

Location:  Jewish Community Center 2 Millstone Campus Drive, Creve Coeur, MO 63141  (Upper parking lot in the rear of the building)

 We will have candy, movie, snacks and plenty of fun!

Photograph Release

I understand that photographs will be taken during this event. I grant permission to the Freddie Ford Family Foundation to use the likeness and image of myself and family for marketing purposes related to the Foundation. My signature below is for the express purpose of granting this permission. 

Assumption of Risk

I understand that participating in the Trunk-or-Treat Event may involve physical activity, crowd movement, tools or equipment use, and other potential hazards. I choose to take part in this event voluntarily. I know there are risks, including personal injury, illness, or property damage. I accept all risks of participation.

Release of Liability

I release and promise not to sue Freddie Ford Family Foundation, its directors, officers, employees, volunteers, vendors, the Jewish Community Center and agents from any and all claims. This includes any injury or damage I suffer due to the ordinary negligence of the organization, other participants or its people during the event and while on the premises.
