I, the undersigned, having joined the Union, hereby authorize and request you to deduct an amount equal to 2.5% of my monthly basic salary and remit such amount as my monthly union dues to the Transport Workers Union of Kenya (TAWU), in compliance with Section 48 of the Labour Relations Act No. 14 of 2007. Also deduct Kshs. 150 of my monthly salary and remit the same to the Central Organisation of Trade Unions Kenya (COTU-K) as affiliation fee. The first deduction is to be made on the next pay day upon receipt of this order.
