PLEASE NOTE

A separate form must be submitted for each game that you are requesting to reschedule.
The rescheduling request must be received at least 8 days before the game is scheduled to be played.
The reschedule request form CLOSES before play in Week 6.
The opposing team must agree to the rescheduling of the game.
There is a $50 rescheduling fee, which must be paid when submitting the request.
If the game CANNOT be rescheduled:the rescheduling fee will be refunded
the game will be forfeited in favor of the opponent.
