LIFE SUMMIT LIABILITY WAIVER AND MEDIA RELEASE FORM

LIABILITY WAIVER AND RELEASE

I, the undersigned participant, acknowledge and agree to the following:

Voluntary Participation: I am voluntarily participating in LIFE Summit (the "Event") hosted by Agape Freedom Fighters and held at The Bridge Community Church.
Assumption of Risk: I understand that participation in the Event may involve certain risks, including but not limited to: physical injury, emotional distress, property damage, exposure to communicable diseases (including COVID-19), and other unforeseen hazards. I voluntarily assume all risks associated with my participation in the Event.
Release of Liability: I hereby release, waive, discharge, and covenant not to sue Agape Freedom Fighters, The Bridge Community Church, their officers, directors, employees, volunteers, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in the Event or while on the premises where the Event is being conducted.
Medical Treatment: I authorize the Released Parties to obtain emergency medical treatment for me if necessary. I agree to be financially responsible for any medical bills incurred as a result of emergency medical treatment.
Personal Property: I understand that Agape Freedom Fighters and The Bridge Community Church are not responsible for any lost, stolen, or damaged personal property during the Event.
Compliance with Guidelines: I agree to comply with all rules, guidelines, and instructions provided by Event organizers and staff. I understand that failure to comply may result in removal from the Event without refund.

MEDIA RELEASE AND CONSENT

I hereby grant Agape Freedom Fighters and its authorized representatives permission to use my name, likeness, image, voice, and/or appearance in any photographs, video recordings, audio recordings, or other media (collectively, "Media") captured during LIFE Summit.

I understand and agree that:

Use of Media: The Media may be used by Agape Freedom Fighters for promotional, educational, informational, and ministry purposes, including but not limited to: website content, social media posts, promotional materials, newsletters, presentations, and future event marketing.
No Compensation: I will receive no compensation, financial or otherwise, for the use of the Media.
Rights Granted: I grant Agape Freedom Fighters perpetual, worldwide, royalty-free rights to use, reproduce, distribute, display, and create derivative works from the Media in any format or medium now known or hereafter developed.
No Approval Rights: I waive any right to inspect or approve the finished Media or any materials in which the Media may be used.
Release of Claims: I release Agape Freedom Fighters from any claims, damages, or liability arising from the use of the Media as described herein.

ACKNOWLEDGMENT AND SIGNATURE

I have read this Liability Waiver and Media Release Form in its entirety. I fully understand its contents and voluntarily agree to its terms and conditions. I understand that this is a release of liability and a contract between myself and the Released Parties.

FOR MINORS UNDER 18 YEARS OF AGE

I, the parent or legal guardian of the minor named above, have read and understood this Liability Waiver and Media Release Form. I hereby give my consent for the minor to participate in the Event and agree to the terms and conditions set forth above on behalf of the minor. I assume all risks and liabilities associated with the minor's participation.
