Termination Request Form

Termination requests are reserved for canceling one’s DCD membership.  Complete and submit this Termination Request Form to the Board via email board@iswimdcd.com at least 15 days before the requested effective termination date.

Submitting this form does not excuse outstanding balances or volunteer hours on a swimmer’s account.  All balances must be cleared before an account can be canceled. Monthly membership fees are not prorated, regardless of when the termination request is submitted during the month.

We highly recommended that you speak with your swimmer group's coach prior to submitting this form to specify the reasons for leaving the team.

Terminations take effect immediately once account balances are cleared and verified.

Do NOT use this form for holds. This form is for requesting the termination of one's DCD membership only.

Acknowledgment: By signing this form, I agree to terminate my membership with DCD, including accounts and access to the swim team. All invoice balances and volunteer hours will need to be cleared and verified by the Parent Board to fully cancel a membership. I understand that upon leaving DCD, swimmers are not guaranteed access back onto the team if they decide to return. The Head Coach and Parent Board may require swimmers to tryout, and reserve the right to refuse re-admittance onto the team for any reason.
