I, the undersigned participant, or parent/legal guardian if the participant is under 18 years of age, acknowledge and agree to the following terms in consideration for participation in the Next Edge Hockey Camp (“the Camp”), operated by Next Edge Hockey, its founders, coaches, staff, volunteers, and affiliates.
1. ASSUMPTION OF RISK
I understand that participation in ice hockey activities involves inherent risks, including but not limited to: bodily injury, illness, permanent disability, paralysis, death, collisions with other participants, contact with boards or equipment, falls, and injuries caused by ice conditions. I knowingly and voluntarily assume all risks, both known and unknown, associated with participation in the Camp.
2. RELEASE AND WAIVER OF LIABILITY
I hereby release, waive, discharge, and covenant not to sue Next Edge Hockey, its founders, directors, officers, coaches, employees, volunteers, sponsors, facility operators, and affiliated organizations from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage sustained while participating in the Camp, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. MEDICAL CONSENT AND RESPONSIBILITY
I certify that the participant is physically fit and has no medical condition that would prevent safe participation. In the event of an injury or medical emergency, I authorize Next Edge Hockey staff to obtain emergency medical treatment if deemed necessary. I understand that I am solely responsible for any medical expenses incurred as a result of participation.
4. INSURANCE ACKNOWLEDGMENT
I acknowledge that Next Edge Hockey does not provide individual medical insurance coverage for participants and that it is my responsibility to carry adequate personal medical insurance.
5. CODE OF CONDUCT & REMOVAL
I understand that participants are expected to follow all Camp rules and instructions. Next Edge Hockey reserves the right to remove any participant for unsafe behavior, misconduct, or failure to follow instructions, without refund.
6. PHOTO & VIDEO RELEASE
I grant permission for Next Edge Hockey to use photographs, video recordings, or likenesses of the participant taken during Camp activities for promotional, marketing, or educational purposes, without compensation.
7. SEVERABILITY
If any portion of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
8. GOVERNING LAW
This agreement shall be governed by and interpreted in accordance with the laws of the state in which the Camp is held.
Next Edge Refund Policy:
We understand that plans can change, and we strive to be as fair and transparent as possible with our refund policy.
-Cancellations made 60 days or more before the start date of the camp will receive a partial refund (90%) of the registration fee.
-Cancellations made between 30 and 59 days before the start date will receive a 50% refund of the registration fee.
-Cancellations made less than 30 days before the start date are non-refundable, as staffing, ice time, and equipment costs will have already been finalized.
-All refund requests must be submitted in writing via email to the Next Edge Hockey staff, including the participant’s name and reason for cancellation. No refunds will be issued for no-shows, late arrivals, or early departures from camp.
By Signing this you acknowledge and confirm both the waiver above and our refund policy and you agree that you are the parent or legal guardian of the above named camper, and are over the age of 18. In case of medical emergency or general medical care, I give consent for medical treatment for the above named camper by authorized personnel. I certify that my child has my permission to attend camp and participate in all activities.
