Workshop Waiver & Media Consent

By participating in the Aerial Workshop and/or Stage Ready Hair & Makeup Workshop, I understand and acknowledge that workshop activities may involve certain risks.

For the Aerial Workshop, I understand that aerial activities, including Lyra and silks, involve physical activity and an inherent risk of falls, injury or accidents. I acknowledge and accept these risks and agree that the workshop instructors and Elements Theatre Co. are not liable for any injury, accident, loss or damage that may occur during the workshop, except where liability cannot legally be excluded.

For the Stage Ready Hair & Makeup Workshop, I understand that various hair and makeup products, tools and equipment may be used. I agree to advise the workshop instructors of any known allergies, sensitivities or other relevant concerns prior to participation.

I confirm that my child is able to participate in their selected workshop and that I have disclosed any relevant injuries, medical conditions, allergies or additional needs that may affect their participation.

In the event of an emergency, I authorise workshop staff to seek appropriate medical assistance or treatment for my child if I cannot be contacted.

No refunds will be issued unless the booking is cancelled with at least 24 hours' notice.
Photo & Video ConsentI understand that photographs and videos may be taken during the workshops for promotional purposes, including Elements Theatre Co. and workshop instructor social media, websites and future workshop advertising.
