Terms and conditions set out in this document provide a contract between CEDA, Calmer Lives and its participants. Prior to booking first session participants must read and acknowledge these terms. For any individual unable to access this form an alternative document will be provided via email or post. The contract will commence from the point it is signed and submitted. Service charges are subject to inflationary increase. Either party may terminate this agreement by giving 4 weeks written notice to the other party.
