PARENT/GUARDIAN MEDIA CONSENT
I am the parent or legal guardian of the minor student-athlete identified above.
I voluntarily authorize ATHLTZ SPK, its owners, employees, contractors, producers, photographers, videographers, editors, representatives, licensees, successors, and assigns ("ATHLTZ SPK") to photograph, film, videotape, record, and/or interview my child in connection with ATHLTZ SPK sports coverage, programming, productions, events, and related activities.
I grant ATHLTZ SPK permission to use my child's name, image, likeness, appearance, voice, statements, interview responses, athletic performance, and other media captured with permission ("Media") for ATHLTZ SPK's editorial, entertainment, informational, promotional, and related media purposes.
This may include use in:
ATHLTZ SPK websites and digital platforms
Instagram, TikTok, YouTube, and other social-media platforms
Original ATHLTZ SPK shows and series
Athlete interviews and features
Game-day and sideline coverage
Highlight and recap content
Documentary and unscripted programming
Podcasts and digital programming
Promotional materials for ATHLTZ SPK and its programming
Broadcast, streaming, and other media platforms
I understand that this content may be publicly accessible and may be shared, reposted, distributed, promoted, or viewed worldwide.
EDITING
I authorize ATHLTZ SPK to edit, crop, excerpt, caption, format, combine, reproduce, or otherwise adapt the Media for legitimate editorial, creative, promotional, technical, and distribution purposes, provided that ATHLTZ SPK does not intentionally use the Media in a materially false or defamatory manner.
OWNERSHIP
I understand that ATHLTZ SPK owns the recordings, photographs, footage, interviews, edits, graphics, productions, and other content it creates, subject to rights in any third-party material.
I understand that neither I nor my child will have the right to inspect or approve finished content prior to publication unless ATHLTZ SPK separately agrees otherwise in writing.
COMPENSATION
I understand that my child's participation does not entitle my child or me to payment, royalties, residuals, or other compensation unless ATHLTZ SPK enters into a separate written agreement providing otherwise.
SPONSORSHIP & ADVERTISING
I understand that ATHLTZ SPK programming and coverage may contain advertising, sponsorships, brand integrations, or other monetization.
This permission does not authorize ATHLTZ SPK to falsely state or imply that my child personally endorses a third-party product, company, political position, or service without additional appropriate authorization.
PERSONAL INFORMATION
ATHLTZ SPK should not publicly disclose the minor's private contact information through content produced under this release. Parent/guardian email addresses, telephone numbers, home addresses, and other private contact information collected through this form are for administrative and communication purposes and are not intended for publication.
VOLUNTARY PARTICIPATION
I understand that my child's participation is voluntary.
I represent that I am the child's parent or legal guardian and have authority to grant the permissions contained in this release.
