  • PHOTO RELEASE I hereby grant Riverside Flight Center (RFC) permission to photograph and/or video me during aviation camp. I authorize RFC to use these photographs, videos, and recordings for promotional, marketing, advertising, educational, and social media purposes, including but not limited to the RFC website, Facebook, Instagram, TikTok, YouTube, printed materials, and other promotional media.*
  • Parent/Guardian Consent for Aircraft Education & Flight Activities

  • As the parent/guardian of a minor, I give my full consent to allow him/her to fly in, fly, or operate a light aircraft. I understand that flying in, flying, and/or participating in activities with a light aircraft involves certain hazards or inherent risks (including injury or death) that cannot be eliminated completely, regardless of the care taken. I am therefore assuming all risk of injury or death and allow the minor to fly in, fly, paticipate in activities, or operate a light aircraft. This choice is made freely and voluntarily, with full knowledge of the nature of flying in, flying, participating, activities, or operating a light aircraft and the risks involved. I agree for the minor to abide by all rules and regulations imposed by the FAA and/or the Riverside Flight Center staff both in and out of the aircraft.

  • Tulsa Riverside Airport Air Traffic Control Tower Visit

  • I give my consent for my child to visit to the Tulsa Riverside Airport Air Traffic Control Tower as part of the Aviation Camp program.
    I understand that the purpose of this visit is to provide campers with an educational, behind-the-scenes introduction to air traffic control, aviation safety, airport operations, and careers in the aviation industry.
    I understand that participation in the tower visit may involve walking through airport facilities, entering controlled or restricted areas as permitted by airport/FAA personnel, and following security and safety procedures established by the facility.
    Parent/Guardian Acknowledgment
    I understand and agree that my child must:
    ● Follow all instructions provided by Riverside Flight Center staff, airport personnel, and air
    traffic control personnel.
    ● Remain with the assigned camp group and designated staff members.
    ● Follow all airport security and safety requirements.
    ● Not enter any area unless specifically authorized.
    ● Not touch equipment or aviation systems unless expressly permitted.
    ● Follow all photography, cell phone, and electronic-device restrictions established by the
    facility.
    ● Behave respectfully and appropriately throughout the visit.
    I understand that Riverside Flight Center does not control the policies, procedures, security
    requirements, or operations of the Tulsa Riverside Airport Air Traffic Control Tower and that the
    visit is subject to the requirements of the airport and tower personnel.

    Release & Assumption of Risk
    I understand that participation in an airport/tower visit involves ordinary risks associated with
    traveling through and visiting an active aviation facility, including walking on airport property,
    transportation to and from the facility if applicable, and exposure to normal airport operations.
    To the fullest extent permitted by applicable law, I agree to release and hold harmless Riverside
    Flight Center, its owners, employees, instructors, and representatives from claims arising from
    my child's participation in the tower visit, except to the extent caused by conduct that cannot
    legally be waived or released.
    I understand that this release does not replace or limit any safety requirements imposed by the
    airport, FAA, or other authorities.

  • Consent for Minor to Operate Unmanned Aerial Vehicle (UAV)

  • As the parent/guardian of a minor, I give my full consent to allow him/her to fly or operate an unmanned aerial vehicle (UAV). I understand that flying or operating UAVs is dangerous and involves certain hazards or inherent risks
    (including injury or death) that cannot be eliminated completely, regardless of the care taken. I am therefore assuming all risk(s) of injury or death and allow the minor to fly or operate UAVs. I agree for the minor to abide by all rules and regulations imposed by the FAA and/or the Riverside Flight Center staff both in the classroom and the operational environment. This choice is made freely and voluntarily, with full knowledge of the nature of flying or operating a UAV and the risks involved therein.

  • GENERAL RELEASE & WAIVER

  • IMPORTANT: This general release does not replace separate consent required for aircraft flights, drone activities, ATC/tower visits, transportation, photography/video, or other activities.

    1. ASSUMPTION OF RISK

    I understand that Aviation Camp may include classroom instruction, aircraft and cockpit familiarization, flight simulators, drone education and demonstrations, walking on airport property, visits to airport facilities, and other aviation-related activities. I understand these activities and the airport environment involve inherent risks, including slips, trips and falls; weather; moving vehicles, aircraft, propellers and equipment; noise; unfamiliar surroundings; and other risks that may not be anticipated. I voluntarily accept these risks on behalf of myself and my child/ward to the extent permitted by law.

    2. RELEASE AND WAIVER

    To the fullest extent permitted by applicable law, I, on behalf of myself and my child/ward, release and agree not to hold responsible Riverside Flight Center, its owners, officers, employees, instructors, contractors, volunteers, agents, affiliated entities, and participating facilities (the “Released Parties”) for claims arising from participation in Aviation Camp, including injury, illness, property damage, or loss, except to the extent such release is prohibited by law. This release applies to ordinary negligence only to the extent permitted by applicable law and does not waive rights that cannot legally be waived.

    3. MEDICAL TREATMENT AUTHORIZATION

    In an emergency, I authorize Riverside Flight Center staff to obtain reasonable emergency medical assistance for my child/ward when I cannot be reached promptly. Riverside Flight Center is not a medical provider. I understand that emergency care may be provided by qualified medical professionals or emergency responders and that I am responsible for medical costs or expenses incurred on behalf of my child/ward, to the extent permitted by law.

    4. CAMP RULES AND SAFETY

    My child/ward must follow all Riverside Flight Center instructions, safety rules, airport rules, instructor directions, and facility/security requirements. Riverside Flight Center may remove a participant from an activity or camp for unsafe, disruptive, threatening, or inappropriate behavior. Activities may be modified, postponed, or canceled because of weather, aircraft availability, airport operations, facility restrictions, staffing, safety concerns, or other circumstances.

    5. PARTICIPATION AND TRANSPORTATION

    This general release does not by itself authorize my child/ward to participate in an actual aircraft flight, operate a drone, enter restricted areas, or ride in transportation provided by Riverside Flight Center. Separate permissions, releases, eligibility requirements, and/or forms may be required.

    6. PARENT/GUARDIAN ACKNOWLEDGMENT

    I certify that I am the parent or legal guardian of the participant named above and have authority to provide this consent. I have read and understand this General Release & Waiver and have had an opportunity to ask questions. I understand that I am signing voluntarily and that this document may affect legal rights.
