PLEASE READ CAREFULLY. 

This agreement applies to all Maven Movement classes, workshops, rehearsals, performances and related activities connected to the program or programs for which I am registering, including activities held at third-party facilities.

I acknowledge that participating in dance classes, workshops, rehearsals, performances and related activities offered or organized by Maven Movement (collectively, the "Dance Program") involves inherent and other risks. These risks may include, but are not limited to, slips, trips, falls, collisions, physical contact with other participants or objects, strenuous physical activity, repetitive movement, overexertion, exposure to communicable illness, and strains, sprains, fractures, aggravation of a pre-existing condition, serious injury, disability, death or property loss.

I freely accept and assume all risks associated with my participation in the Dance Program, whether known or unknown and whether arising from my own actions, the actions of others, the condition of the premises or equipment, or otherwise.

To the fullest extent permitted by law, I waive, release and forever discharge Ashley Brodeur, carrying on business as Maven Movement, together with Maven Movement's instructors, employees, independent contractors, volunteers, representatives and agents, and the owners, operators and occupiers of any premises used for the Dance Program (collectively, the "Released Parties"), from all claims, demands, actions, causes of action, damages, costs or liabilities arising from or connected with my participation in the Dance Program.

This release includes claims arising from the negligence of a Released Party or from a breach of any statutory or other duty of care, including any duty under Alberta's Occupiers' Liability Act.

I agree not to bring or maintain any claim or legal proceeding against the Released Parties concerning any matter released above.

I understand that my participation is voluntary and that I may pause, modify or stop an activity at any time. I agree to follow reasonable safety instructions and to notify an instructor as soon as reasonably possible of any injury or immediate safety concern that arises during the Dance Program.

If Maven Movement reasonably believes that I require urgent assistance, I authorize its instructors or representatives to provide basic first aid, contact emergency medical services and contact the emergency person identified on this form. I understand that I am responsible for any costs associated with emergency medical care that are not otherwise covered.

I am responsible for my personal belongings. To the fullest extent permitted by law, the Released Parties are not responsible for the loss, theft or damage of personal property brought to a Dance Program activity.

Is there anything you would like us to know about your access needs or what would support your participation in Maven Movement? This might include movement considerations, communication preferences, sensory needs or other accommodations. Please share only what you are comfortable sharing.

Information provided here will only be shared with instructors or Maven Movement team members who need it to support your participation.

The personal information collected through this form will be used to administer my participation, support any access needs I choose to identify, maintain waiver records and respond to emergencies. It will only be collected, used, disclosed and retained to the extent reasonably required for those purposes.

Granting or declining this consent will not affect my ability to participate in the Dance Program.

I consent to Ashley Brodeur, carrying on business as Maven Movement ("Maven Movement"), collecting photographs and video or audio recordings in which I appear, whether captured by Maven Movement or provided by me (collectively, the "Media").

I authorize Maven Movement to use and disclose the Media for the purpose of documenting and promoting its programs, performances and community. This may include Maven Movement's website, social-media accounts, newsletters, posters, brochures, advertisements, promotional videos, dance clips and other print or digital promotional materials. My first name or social-media handle may be included when I have provided it to Maven Movement for that purpose.

I understand that:

• The Media may be cropped or edited for format, length or clarity but will not intentionally be used in a way that misrepresents me.
• I will not receive payment or other compensation for an authorized use.

• Media published online may be viewed, copied or shared by people outside Maven Movement's control.
• Maven Movement will not sell or license the Media for an unrelated third party's independent advertising.
• Maven Movement may provide the Media to photographers, videographers, designers, printers, venues and platform providers working on Maven Movement's behalf.
• I may withdraw or change my consent for future use by contacting Maven Movement in writing. Maven Movement will stop new promotional uses and, where reasonably practicable, remove the Media from channels it controls. It may not be possible to retrieve materials already printed, distributed or copied by others.

If I participate in a public performance, I understand that audience members, venues or other third parties may photograph or record the event. Maven Movement cannot guarantee that I will not appear in recordings created or shared by people outside its control.

If no option is selected, Maven Movement will treat this as declining promotional consent.
