I acknowledge that participating in the Event involves inherent risks, including but not limited to physical injury, illness (such as communicable diseases like influenza or COVID-19), emotional distress, and, in extreme cases, permanent disability or death. In consideration for being permitted to participate in this event, I, on behalf of myself and my heirs, assigns, personal representatives, and next of kin, release and hold harmless Metro Church, Crosswalk Fellowship, and their elders, pastors, staff, leaders, officers, employees, agents, and volunteers (collectively, the “Released Parties”) from claims, losses, damages, or liabilities arising out of or related to my participation in the event, including personal injury, death, or property damage, to the fullest extent permitted by law.
This release includes claims arising from the ordinary negligence of the Released Parties but does not apply to gross negligence, reckless misconduct, or any liability that cannot legally be waived.

However, I understand and agree that Metro Church is not responsible or liable for any medical expenses incurred as a result of my participation in the Event.

If I choose to drive independently or arrange my own transportation to or from the event, I understand that this decision is voluntary and made at my own discretion. I acknowledge that Metro Church, Crosswalk Fellowship, and their staff, leaders, volunteers, and affiliates do not control or assume responsibility for independently arranged transportation, including:

• The condition, safety, or maintenance of the vehicle.
• The driver’s actions, qualifications, or compliance with applicable laws.
• Accidents, injuries, damages, or losses that may occur while using independently arranged transportation.

I understand that I assume the risks associated with transportation that I independently arrange or choose to use.

I hereby grant Metro Church permission to use my likeness in photographs, videos, or other digital media ("photos") for any and all publications, including web-based materials, without payment or other compensation.

I understand and agree that all photos will become the property of Metro Church and will not be returned. I irrevocably authorize Metro Church to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. I also waive the right to inspect or approve the final product in which my likeness appears, as well as any right to royalties or compensation related to the use of these materials.

Furthermore, I release and forever discharge Metro Church from any claims, demands, or causes of action that I, my representatives, executors, administrators, or any other persons acting on my behalf may have in connection with this authorization.
