TIMING FOR YOUR EVENT

Please allow enough time when planning your event. We recommend 15 minutes per person, plus about 1 hour before and after the event for setup, cleanup, and travel. For example, 30 people would require at least 7.5 hours of massage time alone, or about 10 hours total. Larger events may take several days, and we will try to provide a second massage therapist if possible. Depending on the number of people, you may need more than one day for your event.

We also recommend allowing 2 weeks to 1 month for scheduling so we can best accommodate your event.

RAFFLE

Vitality Health Massage will offer a raffle prize of a free 60-minute massage and a free 90-minute massage for your guest or staff member.

DOCUMENTS

We will send consent forms for your guests or staff to sign, a service contract, W-9 and 1099-NEC tax forms, a photo authorization form, and, if needed, a sign-up sheet to help you organize staff or guests for chair massage. We can also provide a flyer template for your event so you can advertise it and hang flyers around your workplace to help spread the word or use them for marketing purposes.

PAYMENT

The price shown on this form is an estimate provided for quoting purposes only and is not guaranteed. The final price will be determined once the event details, contract, and headcount are confirmed. Payment will be due when you accept the finalized agreement. We typically send an invoice through Square or Stripe, which can be paid by credit or debit card.
