Please Read Carefully Before Signing
I understand that participation in STS Elite basketball tryouts involves physical activity and carries inherent risks, including but not limited to falls, collisions, contact with other participants, muscle strains, sprains, fractures, head injuries, other bodily injuries, and, in rare cases, serious injury or death.
In consideration for being permitted to participate in STS Elite tryouts at Monsignor Scanlan High School, I knowingly and voluntarily assume all risks associated with participation, whether known or unknown, and accept full responsibility for any injury, illness, loss, or damage that may occur as a result of my participation.
To the fullest extent permitted by law, I agree to release, waive, and hold harmless STS Elite (Students to Stars, Inc), its owners, directors, coaches, staff members, volunteers, representatives, agents, and affiliates, as well as Monsignor Scanlan High School and its respective employees, representatives, and agents, from claims, demands, damages, liabilities, or causes of action arising from or related to my participation in the tryout, including claims resulting from ordinary negligence, except where such release is prohibited by law.
I further acknowledge that I am physically capable of participating in basketball activities and understand that STS Elite does not guarantee the availability of medical personnel during the tryout. In the event of an emergency, I authorize STS Elite personnel to obtain or arrange reasonable emergency medical treatment when necessary. I understand that I or my parent/legal guardian will be responsible for any medical expenses resulting from such treatment.
By signing this waiver, I acknowledge that I have read and understood these terms and voluntarily agree to assume the risks associated with participation in STS Elite basketball tryouts.
