  • Is IHM the right fit?

    IHM’s scope and style of care are intentionally specific. The following questions help us identify when another practice or provider may be better equipped to offer the care requested.
  • Is the prospective patient 18 or older?*

  • IHM generally provides care for adults. Patients under 16 fall outside our current scope of practice.

  • Are you currently seeking diagnosis or treatment for active cancer?*
  • IHM does not diagnose or treat active cancer. An oncology team is best equipped to direct this care. If you are seeking integrative care for a separate health concern, you may continue with this form while maintaining the appropriate oncology care.
  • Are you seeking a provider to prescribe or manage a controlled medication?*
  • IHM does not prescribe or manage controlled medications. If this is the primary care you are seeking, IHM will not be the right fit. If you are seeking integrative care for a separate health concern, you may continue with this form while maintaining the appropriate prescribing provider.
  • Are you looking for IHM to assume management of an extensive or medically complex prescription regimen?*
  • To ensure that we can provide care within the scope of our integrative approach, IHM providers do not prescribe or manage medication regimens involving more than five pharmaceutical medications or more than two psychiatric medications.
  • Do you currently have a primary care provider?*
  • In some cases, patients may be asked to maintain their existing primary care provider and utilize Integrative Health Matters as a complementary part of their care. Determination of eligibility for primary care services will be made by the provider during the new patient visit.

  • How care works at IHM

    Care at IHM is collaborative. Depending on your care plan, patients may work with physicians, family nurse practitioners, wellness consultants, health coaches, and other qualified members of the IHM team. The team coordinates care so that each part of the plan is supported by the person best equipped to provide it.
