Pumpkin Fest 2026 Contact and Permission Form

All Pumpkin Fest Volunteers are required to have an Emergency Contact on file with us who we can call if you have an issue while in the park. For those under 18, we also need get formal permission from a parent or guardian for you to participate in Friends of Silvermont's Pumpkin Fest setup or Event Night activities. You may fill out the Emergency Contact part yourself, but please have your parent or guardian review and sign the permission part of this form.
By signing, they will be verifying that they understand the following:
- that youth volunteers are required to check in and check out at the volunteer tent and that failure to check out at the end of a shift will result in a phone call to the volunteer's emergency contact
- that students will be given orientation and training necessary for the safe and responsible performance of the volunteer duties and will be expected to meet all the requirements of the position.
- that students will receive no monetary compensation from Friends of Silvermont for this work.
- that inherent risks may be associated with volunteer activities, and that they will not hold Friends of Silvermont accountable or liable for any injuries that unintentionally result from the child’s participation, or that arise during time spent volunteering due to any underlying physical condition.
