Medical Information

The information you provide will be used to help keep your child safe and well during Listooder meetings. Those in leadership of the youth fellowship will hold this information and, if necessary, it may be shared with other leaders e.g the first aider. In an emergency it will be shared with medical staff.

Permissions

I (having parental/guardian responsibility for the above named person) give consent and permission to the following:
Any emergency medical treatment that may be necessary during the duration of the Listooder activity.
Some photographs and video taken on Listooder activities are used in publicity materials and on the Listooder Website, including Social Media e.g. Instagram & Facebook.
I give permission for my child to be transported in either a private car or other transport provided by Listooder Youth Fellowship.
I give permission in the event of illness or accident for my child to be treated by a nominated first aider or by a qualified medical practitioner. However I understand that every effort will be made to contact me as soon as possible.
