• Fluoroscopy Training Knowledge Check Form

  • Before completing the Fluoroscopy Training Knowledge Check, please be sure to review the Fluorscopy training videos found here: https://www.providence.org/locations/ak/alaska-medical-center/for-physicians/fluoroscopy. [link will open in a new tab]

Once the training videos have been reviewed, please return to this form to attest to reviewing and move onto the Knowledge Check portion. If there are any questions or concerns, please reach out to us.

  • Thank you for reviewing the fluoroscopy training videos. Please complete the knowledge check below. To pass the knowledge check, you must answer 12 out the 15 questions correctly. If you do not pass this threshold, you will be asked to complete the knowledge check again immediately. If you do pass, you will receive a notification of your score and that your training has been completed. You will have a maximum of 3 attempts to pass the threshold for the knowledge check. If you are unable to pass the threshold after 3 attempts, then Alaska Medical Staff Services will require you to complete a Fluoroscopy Training course and provide a certificate of completion.

If you have any questions or concerns, please reach out to AKMedicalStaff@providence.org.
