Please complete this application to be considered for a table at the Food is Medicine Beehive, part of the second annual WVFIMC Summit, taking place October 26 at Stonewall Resort in Roanoke, West Virginia.

The event is free to participate, but space is limited. Applications close September 30, and selected organizations will be notified by October 5.

Tables will be provided. Please bring any materials you would like to use to share your work, such as handouts, posters, or other resources.
