Boys Soccer PERMISSION FORM
If your son is interested in joining the 2026-27 school soccer team, please complete the form by Monday, September 21.

CRITERIA for joining and staying on the team:

1. Ability to attend most practices and games.

2. Rides to games are to be arranged by parents.

3. Consistent effort and attention to all school work.

4. Correct soccer uniform, cleats and shin pads must be worn to all games and practices.

5. A positive sportsmanlike attitude towards coaches, referees and players.
All divisions, including senior, will be playing Super Eight format.

Practices will be at Star for all teams, and they will be Mondays 3:00-4:15 for all teams.

Grade 5 games: Tuesdays

Grade 6 games: Thursdays

Grade 7 games: Thursdays

The season will begin the week of September 21 with practices and finish by the end of October.

REMINDER: siblings of soccer players are not permitted to stay after school without parent/adult supervision
